As per coronavirus preparedness and response Supplemental Act, as signed into law on March 6, 2020, Tele health services are provided during COVID-19 PHE. This visit is performed via telemedicine. The patient consents to conduct this visit via video conferencing/ telephone technology. The patient’s identity has been confirmed. This visit was conducted directly between the patient and the provider listed. The patient initiated the contact leading to this visit. The findings during the visit are based on the patient’s best ability to communicate symptoms and without a physical examination.
