Your doctors have recommended hospitalization in order to treat and monitor your current illness. Our goal is to provide you with the best medical care possible within a respectful and compassionate setting.
YOUR PRIMARY CARE DOCTOR
Your primary care doctor will assign Hospitalist Physicians to care for you during your hospital stay. Your primary care doctor is an important member of your health care team and will resume responsibility for your medical care once you leave the hospital.
WHAT IS A HOSPITALIST PHYSICIAN?
During your stay you will be examined and treated by a variety of specialists. Among them will be a team of doctors who will identify themselves as Hospitalist Physicians. A Hospitalist Physician is a medical doctor who specializes in the evaluation and treatment of patients who require hospital care.
WHAT DOES A HOSPITALIST PHYSICIAN DO?
A Hospitalist Physician spends 100 percent of his or her time caring only for hospitalized patients such as yourself. Hospitalist Physicians function as your primary care doctor during your stay at the hospital. They work with other specialists to monitor, guide and coordinate all major aspects of your treatment. A member of the hospitalist team will visit you daily, assist with your medical care, discuss medical issues with you and your consulting specialist physicians, and arrange for follow-up with your community-based primary care doctor upon your discharge.
WHAT IF I NEED ANOTHER SPECIALIST DURING MY HOSPITAL STAY?
It is very likely that you will be seen and treated by a variety of specialists during your hospitalization. Specialist evaluation and treatment is a covered benefit through your insurance.
WHO DO I TALK TO IF I HAVE ANY QUESTIONS?
Your Hospitalist Physician is responsible for directing and coordinating your medical care. He/she is available to answer any questions or concerns you or your family may have about your condition and proposed treatment plan. Your case manager may also be of help in answering questions about your insurance benefits or home care.
WHAT IS A CASE MANAGER?
A case manager is a licensed nurse who is experienced in assisting a patient through the hospital stay. He/she will work closely with your Hospitalist Physician to identify and meet any special needs you may have both during your hospitalization and the time of your discharge.
WHAT DOES A CASE MANAGER DO?
The case manager will arrange for after-hospital needs that are covered by your insurance such as convalescent care, home health a class="readtext"ssistance and medical equipment or other services your doctor may order. The case manager will also explain to you what services are covered by your insurance.
WILL MY PRIMARY CARE DOCTOR BE INFORMED ABOUT MY ILLNESS & TREATMENT?
Your primary care doctor will receive important information about your hospital diagnosis and treatment in the form of a written discharge summary. The report will be sent to your physician soon after you are released from the hospital. In addition, Hospitalist Physicians and primary care doctors may discuss your case over the phone in order to facilitate your transition from the hospital to the care of your primary care physician.
WHAT HAPPENS AFTER I AM DISCHARGED FROM THE HOSPITAL?
You will receive written and verbal instructions about your home treatment plan. You should make an appointment to see your primary care doctor within one to two weeks after you are released from the hospital. Your Hospitalist Physician and case manager will provide further information and answer any questions you may have prior to your discharge. If you should have further questions or health problems while you are at home, call your primary care doctor for assistance.
WHAT IF I DON’T HAVE A COMMUNITY DOCTOR?
Your case manager will assist you in selecting a local primary care doctor before you leave the hospital. You may also call your health plan’s customer service representative if you should have further questions about your primary care doctor once you are home.
Admission into the hospital can be confusing and stressful. We hope that the above information will help minimize your apprehension by answering commonly asked questions.
